For your first visit, please bring the following:

• Insurance Card (if you have insurance)
• Proof of Income (required to apply for our Sliding Fee discount program)
• Photo Identification (or an item with your current address)
• All medications that you are currently taking (bring the bottles/containers)

You will be asked to complete documents at your visit. Save time at your appointment by downloading, printing, and completing your new patient registration forms in advance. Visit our Patient Forms page to print the required documents and bring them with you to your visit.
